奥乐齐(中国)投资有限公司以及其关联方（“奥乐齐”或“我们”）承诺严格遵守相关法律、法规以及本《个人信息收集同意书》（“知情同意书”）的规定收集和处理有关您的个人信息。奥乐齐将以合理谨慎和勤勉的方式严格保密地存储您的个人信息。

ALDI (China) Investment Co., Ltd and its affiliates (“ALDI”or “We”) commits to meet the requirements under relevant laws and regulations as well as under this personal informaiton collection consent form(“Consent Form”)to collect and handle your personal information.  ALDI will  store your personal information in strict confidentiality with reasonable care and diligence.

奥乐齐收集您的个人信息是为了履行《单用途商业预付卡管理办法（试行）》项下的法定义务[1]，以满足监管的要求。除此之外我们不会将您的个人信息用作其他的目的。

ALDI collects your personal information in order to fulfil the legal obligations under the Provisional Administrative Measures on Single-purpose Commercial Prepaid Cards and to meet regulatory requirements.  We will not use your personal information for any other purpose.

2.个人信息收集的范围

The Scope of Personal Information collection

我们将通过本《知情同意书》附件（一）中的表格来收集您的如下个人信息：姓名、电话号码、个人有效身份证件[2]，除此之外我们不会收集或处理您的其他个人信息。

We will collect your following personal information through the sheet  in Annex 1 of this Consent Form: Name, Phone number,Valid identity document, and we will not collect your other personal information.

3.向第三方披露

Disclosure to Third Parties

除非获得您的事先许可或是按照相关法律法规的要求进行披露，我们不会向除ALDI以外的任何第三方披露或转让您的个人信息。

 Unless your prior consent is obtained or otherwise required by relevant laws or regulations, we will not disclose or transfer your personal information to any third party other than ALDI.

4.个人信息的储存

The Storage of Personal Information

您的个人信息我们将储存在中华人民共和国境内。我们将会按照适用法律的要求将您的个人信息储存5年。在您的个人信息超出保存期间后，我们会根据法律要求删除或匿名化处理您的个人信息。

Your personal information will be stored within the territory of the People's Republic of China.  We will store your personal information for 5 years as required by the applicable law.  When the storage period expires, we will delete or anonymize your personal information as required by relevant laws.

若您对本《知情同意书》的内容有任何疑问、意见或建议，或您发现我们处理的您的个人信息有错误时，您有权要求我们做出说明或进行更正。您可以通过发送邮件至邮箱giftcard_center@aldi.cn 行使您的权利。我们可能会核实您的身份，并在15个工作日之内回复您的请求。

If you have any questions, comments or suggestions about this Consent Form，or when you spot errors in your personal information processed by us, you have the right to ask us to make clarifications or corrections. You may exercise your rights by sending an email to giftcard_center@aldi.cn . We may verify your identity and will respond to your request within 15 working days.

若您同意我们按照上文所载方式处理您的个人信息，请向奥乐齐工作人员表示“同意”或签署本同意书；若您不同意，您将无法购买您所需的礼品卡。

Please indicate your consent to ALDI staff or by signing this form as indicated below if you consent to our handling of your personal information as set out above.  If you do not agree, you will not be able to purchase the gift card as you request.

[1]《单用途商业预付卡管理办法（试行）》第15条个人或单位购买（含充值，下同）记名卡的，或一次性购买1万元（含）以上不记名卡的，发卡企业或售卡企业应要求购卡人及其代理人出示有效身份证件，并留存购卡人及其代理人姓名或单位名称、有效身份证件号码和联系方式。

Article 15 of Administrative Measures on Single-purpose Commercial Prepaid Cards (Trial Implementation)  Where an individual or organization purchases (including top-up, same hereinafter) a registered card, or makes a one-off purchase of a non-registered card at RMB10,000 or more, the card-issuer or card-seller shall request the card purchaser and its agent to present a valid identity document and retain the name of the card purchaser and its agent or the name of the organization, valid identity document number and contact details.

 

[2]个人有效身份证件包括：居民身份证、户口簿、军人身份证件、武警身份证件、港澳台居民通行证、护照等。

Valid identity documents of an individual include resident identity card, residence booklet, military identity document, armed police identity document, Hong Kong, Macao and Taiwan resident pass, passport, etc.
